near-miss


WHY IS REPORTING A NEAR-MISS IMPORTANT?
The only way to reduce the number of incidents is to
identify near-misses that happen on board is to learn and react to them.
The vast majority of accidents should never have occurred in the first place
and we all need to take the responsibility of playing our parts in
attempting to eliminate them altogether.
Only by identifying near-misses and unsafe acts can we hope reduce the chance
of injury and/or death.
But we cannot do this without reporting when a near-miss happens.
